Important Reminder for Our Returning Patients!

 

To keep everything running smoothly, all returning patients are required to complete our New Office Policies once a year. The updated forms include our HIPAA policy, financial policy, and office policies.

 

If these forms aren’t completed before your appointment, they may cut into your scheduled time—and some services may need to be rescheduled for another day. To avoid any delays, please be sure to complete your forms in advance.

 

If you’re unable to complete them ahead of time, we kindly ask that you arrive at least 15 minutes early to allow enough time for paperwork.
